Film Festival Entry Form

All projects have to be 5 minutes or under. Films can be submitted in the in the following formats: DV (mini DV or DV Cam), DVD or CD (Mpeg or Quicktime movie files only).

To enter this festival/competition, you must complete the following form prior to mailing your short film/video to MY HERO.

Remember: To enter the MY HERO Short Film Festival you must include all neccessary Release Forms. If you or one of your actors is a minor, a Parental Consent Form is required.
